Care Program offers all the warmth of family and all comforts of home in a setting that is both stimulating and secure..
Indeed,
here at Grace Hill, we know that just because an individual with Alzheimer’s or dementia can no longer remember a certain
name, this morning’s breakfast, or how to care for themselves, doesn’t mean they’ve forgotten the joys of
a warm smile and a gentle touch. So here at Grace Hill’s Alzheimer’s/Dementia Program, every aspect of every resident’s
care includes both.
Our
home provides specialized Alzheimer's/dementia programs that emphasize dignity and individuality in an appropriate resident
focused setting. We realize that each person with Alzheimer's/dementia is unique, and therefore each resident's care program
is designed to maximize functional independence by emphasizing the resident's remaining strengths and abilities. We offer
our residents' activities as well as special design features that add to the supportive atmosphere and help to reduce stress
levels and behavioral symptoms.
Cognitive
and Physical Support:
The
support that is provided is more than just a practical solution to certain physical challenges. It is a distinct advantage
for those who prefer to spend their energies enjoying Grace Hill’s many leisure opportunities.
LEVEL ONE - Level I allows for reminders and occasional assistance from staff for appointments, meals, grooming,
showers and medications. Health is monitored routinely so that one may remain as active as possible. Weekly housekeeping and
laundry, therapeutic diets, daily snacks, and social, cultural, spiritual and educational programs, and scheduled transportation
are some of the amenities.
LEVEL TWO - Level II includes all services in Level I, in addition to daily hands-on assistance with showers,
grooming, hygiene, morning and evening dressing. Staff will assist with manageable incontinence. Daily housekeeping is provided
by the staff to assure safety. Medication administration, escort service to meals and activities are provided.
LEVEL THREE - This level provide a specialized service to the frail elderly or seriously ill
resident. Specific medical treatments delegated by the Care Coordinator and Staff include Diabetic supervision or other medical
supervision assistance with medication and procedures.
